# How do I add form fields like checkbox, dropdown and textbox to a task?

To add a form field, select the appropriate task where you would like to add the form fields and click ${color}[#000000](**+Add Form Field**).

A new window will open with form fields where you have the choice of **"Add checkbox"** (1.), **"Add dropdown"** (2.) and **"Add text field"** (3.).

If you have selected "Add checkbox", a new window will open where you can enter the **Title** (1.) and the **Checkbox Description** (2.). You have the possibility to create multiple checkboxes by clicking **+ Add Item** (3.). Once you have added all the checkboxes, select the **Add** button (4.).

If you want to edit these again, click the **Edit button** (1.), otherwise, you can click **Save and Assign** (2.).

To add a dropdown select the corresponding **Add dropdown** button below.

Enter the desired **Title** (1.) and the **answer options** (2.). To create multiple answer options, click **+ Add Item** (3.). Select **Add** (4.) once you have added all the dropdown options.

If you want to add a text field, select **Add text field** below.

Create the **Title** (1.) and then click **Add** (2.).

Finally, select **Save and Assign** to save everything.
